Request for Proposals to Provide Evidenced Based Programming
Buckeye Hills Regional Council is requesting proposals from agencies to provide evidenced based/evidenced informed programming to persons 60 years of age and older or are under 60 years of age who have a disability and reside within the counties of Athens, Hocking, Meigs, Monroe, Morgan, Noble, Perry and Washington.
The primary funding source is; Older American's Act Title-III D.
Services eligible for Title III-D funding are: Evidenced Based/ Evidence Informed programs designed to help older adults prevent and manage chronic diseases and increase healthier lifestyles.
One (1) complete application in hard copy, signed in blue ink, must be received by Buckeye Hills Regional Council at 1400 Pike Street, Marietta, Ohio 45750 or by mail at the same address by 4 p.m. on August 8, 2019. Faxed or e-mailed proposal applications will be rejected. If mailed, bidder is required to use certified return receipt. If hand delivered, bidder will be given a receipt at time of delivery.
