Position: Care Management Coordinator

Responsibilities

This full-time position will focus on Athens or Meigs County service areas. The ideal candidate for this position will utilize their skills and knowledge from their profession as an RN or LSW with minimal direction from their supervisor to implement, evaluate, and coordinate waiver services.

Education/Experience Required

Must be a Registered Nurse or Licensed Social Worker with a valid and current license in the State of Ohio, and one (1) year experience. Fingerprinting and background check required for hire.

We offer an excellent benefits package including mileage reimbursement, paid health, vision, dental, 35-hour work week, generous paid leave, and participation in the Ohio Public Employee Retirement System. Ability to work from home. The minimum starting salary for this position is $30,720.
